To begin your journey with Amelia Underwriters, we require the submission of specific documents for approval. These documents include a completed and signed Broker Profile & Brokerage Agreement, which provides us with vital information about your agency. Additionally, we ask for a copy of your E&O (Errors and Omissions) Declarations page, your Agency License, and a certificate confirming your agency's credentials. It's also essential to provide copies of state licenses for your agency staff, ensuring compliance with all regulatory requirements.

Furthermore, please submit a completed and signed W-9 form, as well as ACH authorization for the processing of return premiums and commissions. These documents help us facilitate efficient financial transactions and maintain accurate records for our mutual benefit.
